How To Pay

Please make cheque payable to CVHA and mail to the CVHA office at P.O. Box 651 Maple Ont. L6A 1S5 upon submission of Sponsorship Form.

Jersey production will not begin until the cheque is received at the CVHA office and the Sponsorship is approved.

Please remember to email your company logo to [email protected]. Ideal formats are .AI or .PDF (vector ). If you don't have this please send the highest resolution .JPG file you have.
